Performance Improvement Consultation Data Request Spreadsheet (Not a U.S. Government Web Site)
This document is a spreadsheet designed to capture specific data elements that have proven effective in the preparation for the consulting team. It also facilitates completion and reply in electronic format. It is designed to help the consulting team analyze information prior to arrival and thus maximize use of time with hospital administration, leadership, department managers, staff and physicians while actually on site. First developed by Stroudwater Associates in 2001 for the Delta RHPI Project.

Performance Improvement Consultations Data and Information Request List (Not a U.S. Government Web Site)
Prior to arrival on site for a performance improvement (PI) consultation, it is essential that the consultants have current information about the hospital and community. This document contains a list of specific data elements that have proven effective to prepare the consulting team and maximize the use of time while on site with physicians, administration and staff. Strategic Management Consultation - One Approach for the Balanced Scorecard (Not a U.S. Government Web Site)
This process tool was developed by Community Healthcare Systems, LLS and James Barnes, Ph.D. Community Healthcare Systems, LLC is headed by Joseph T. Hammond, a former hospital administrator and participant of a Balanced Scorecard consultation. James Barnes is at Louisiana State University and has direct experience implementing BSC with rural hospitals in Oklahoma.

Comparative Employee Feedback Data Aggregate Report (Not a U.S. Government Web Site)
Delta eligible hospitals have the option of participating in an employee feedback assessment. This assessment is available for full and part time hospital employees. An aggregate report of results is included with corresponding questions in six categories: planning, administrative and management, training, environmental and retention, performance and peer issues and supervisory issues. This report can be used to simulate discussion regarding employee issues.
