- Support from the board to submit the application.
- Demonstration of a need for technical assistance in the application and through the follow-up interview(s).
- Demonstration of willingness to participate in the performance improvement process; examine current hospital performance and to consider strategies for improvement; engage fully in change processes; and to share data and information with consultants.
- Recommendation from the state hospital association and/or the state office of rural health that the hospital is a good candidate and would benefit from this service.
- Determination that there are no pending or anticipated issues that would hinder the consultation process.
- Determination that there is a good fit between the consulting organization and the applicant hospital; that the timing is right; and there is a good fit between hospital needs and available consulting services.
- Equity among the eight states so that each state receives project services.
